1 Benefit payment request
The Benefit payment request form provides the Fund with authorisation from the employer that the employee has terminated their employment. The form also provides member details, date of leaving service, the type of benefit to be paid and a summary of the contributions paid for the current financial year.

2 Employer details
Current members of the Fund who are eligible for Super Choice and want their future 9% Superannuation Guarantee (SG) contributions from a new employer/s to be made to CSRF should print and have their employer/s complete this form.
